Purpose:
|
Eye care is an essential component in the management of critically ill patients. The aim of this study was to evaluate the incidence of exposure keratopathy (EK) in intensive care unit patients.

Methods:
|
A prospective, cross sectional study was performed in three general adult ICU in referral hospital. A record was made of age and sex, and GCS and the cause of hospitalization The eyelid position, presence of conjunctival oedema, and degree of EK of 25 patients who had been ventilated for at least 7 days were examined daily by expert nurse. EK was assessed with fluorescein and a pen torch with a cobalt blue filter.

Results:

Twenty-five patients (50 eyes) were included. The mean age of the patients was 53.4±23.8 years. Eighteen (72%) patients were male, 12 (48%) was admitted to ICU for trauma, and 3 (12%) had lagophthalmos. Mean GCS was 7.2±2.6 (range 3-11). EK was seen in 60% (15/25). The incidence of EK increased from the first day (28%) until the seventh (60%). In addition, the severity of EK was increased from the beginning to the end of the study. EK was commoner in patients with incomplete lid closure than in patients with closed lids.

Conclusion:

EK is a common problem in patients hospitalized in the ICU. Unfortunately EK can be seen from the first day of admittion, which gradually increases the incidence and severity. ICU staff should be educated about the value of protecting the eyes of their patients. With application of simple protocols, exposure keratopathy can be prevented, thus improving patient care in the ICU.
