Purpose:
|
Although DVD is usually bilateral ,it is asymmetric in most cases. Some authors suggested classic IO anterior transposition ( AT) whereas others recommended graded IOAT for DVD correction
|

Methods:
|
Cases with DVD >5 pd at least in one eye and difference >5 pd with the fellow eye and also IOOA entered in this clinical trial study . History of strabismus operation ,ocular ,mental ,systemic disorders were excluded . IO of the eye with more DVD were sutured to sclera at the level of IR and IO of the eye with less DVD were sutured 2 mm posterior to IR insertion in graded IOAT ,whereas both IO were sutured at the level of IR to sclera in classic technique .Patients with DVD < 5pd in intensity were considered as success outcome ,3 months postoperatively.

Results:
|
20 cases in Graded IOAT group had DVD difference of 7.25+\-3.6 and 1.27+\-2.16 pre and post operatively and 22 cases in Classic IOAT showed DVD difference of 5.23 +\- 4.39 and 3+\-3.62 pre and post operatively . Although the pre and postoperative difference of each group was significant (p1<0.001 ,p2= 0.002) ,the difference of 2 groups was not significant

Conclusion:
|
Based on our results ,both grading and classic IOAT were effective for correction of asymmetric DVD . The reason may be due to IO muscle ability to get along with it’s new condition post operatively
