Purpose:

To investigate the comparative efficacy and safety of 360° Myoring implantation at three different depths in eyes with keratoconus.

Methods:

In an interventional case series, 53 eyes with keratoconus, intolerant of contact lenses were implanted with continuous intra-stromal ring (MyoRing, Dioptex, GmbH, Linz, Austria) at either 200 (group 1), 250 (group 2) and 300 µm (group 3) depths. An intra-stromal pocket was created using the femtosecond laser and MyoRing was implanted into the pocket. Visual and refractive outcomes, keratometery, central corneal thickness, corneal biomechanical characteristics and higher order aberrations were evaluated at baseline, one, three, six and twelve months after surgery.

Results:

Fifty-three keratoconic eyes of 53 patients were included, Mean patient age was 25.56 ± 6.4 years. In all three groups, uncorrected distance visual acuity significantly improved after MyoRing implantation, but the improvement in the corrected distance visual acuity was not significant in neither group. Spherical and cylindrical parts of refraction and also the mean central keratometery reduced significantly in all three groups at 1-year follow-up visit. Corneal hysteresis and corneal resistance factor did not change significantly after the procedure. Complications were comparable between groups and None of the patients had any intraoperative or postoperative complications. There was no significant difference in any of the measured variables between 3 groups.

Conclusion:

Implantation of Myoring at a depth of 200 and 250 µm yields comparable clinical outcomes as that of 300 µm. Ring implantation at a depth of 200 and 250 µm can be considered in keratoconic eyes with low pachymetry.
